Use either of the buttons below to enroll your client into Part B of Medicare. The top button is the paper application you can fax in. The bottom button is a link to the online application on the Social Security Administration’s website.

United Healthcare

UHC allows you to obtain a remote signature for the Scope of Appointment in the LEAN Application.
UHC allows you to compliantly present the Plan and Summary of Benefits over the phone.
​The beneficiary does not have to have a booklet in hand. You may fill out the application for the beneficiary and then use a remote signature in order to have them sign.

Humana

Humana allows you to obtain the Scope of Appointment using telephonic recording, the phone number is below.
You must email the Digital Summary of Benefits to the beneficiary.
You can access training for this on the Humana MarketPoint University by searching for Digital Marketing Materials.
After making the compliant presentation, you can then email the client a link for the beneficiary to fill out the application.
​The link is connected to your SAN Number with Humana.

Telephonic Scope of Appointment

FastApp Enrollment

You can complete a telephonic enrollment using the FastApp Enrollment App. 
You'll complete the App and, at the end, it will give you the option to complete the IVR. 
When you click that, you will receive the App ID and phone number to call.
You will key in the App ID and confirm the clients name and DOB. After that, the client will complete the rest of the process on their own.
Prepare the client for 10 minutes worth of statements and them saying "Yes" to agree. 
Pro Tip: you can edit the app at any point and call the phone number after even after you've called the phone number and started the process. Just hang up, edit the app, and re-dial.
